RN/Case Manager
Rockbridge Area Hospice has an opening for a full time position as RN Case Manager. The RN Case Manager works as part of an interdisciplinary team in the delivery of end-of-life care and provides training to the hospice patient and family. The ideal candidate has a compassionate character and a willingness to be patient and present with people who are going through their life transition. An inquisitive mind, good communication skills and flexibility area must. Candidates must possess a current Virginia RN license and have at least two years work experience. Home Health and/or Hospice experience preferred.
